UżytkownikAgain for Alex or anyone that can help. I did receive A copy of the „Akt Zgonu” for HIldegrad Magdalina. That document names the person that reported her death as a daughter and provides her married name. I came hoping to find someone that could help me search for that person. She probably lives in Kotowice.I would be will to pay a reasonable fee for the help.

UżytkownikHello Axee. Thanks for your interest and information. I have recently been in email contact with USC Katowice. They confirmed that the record 4545/89 is for Hildegard Magdalina Kolassa. They have agreed to provide me with that document. I am now in the process of providing them with the necessary request form and payment information. The people at USC Katowice have been very friendly and helpful throughout the process

UżytkownikAgain, for Alex. I made an information request to Urząd Stanu Cywilnego in Katowice regarding the death of Hildegard Magdelina Kolassa. I received a reply that there is no record of her death in their electronic database. She would most likely have been married, at least once, if she lived to 86 years of age. I’m not sure that Urząd Stanu Cywilnego would be able to identify her by her maiden name.

UżytkownikTo Alex regarding your post of 7 Mar 2020. I somehow missed this post until today. I think the document and picture you have attached to your message are from the family tree I have on Ancestry – Klecker Family Tree. I see the note at the bottom left of the birth document. I did not understand that this was information about her death. I do not understand how to read that note. I understand „Kotowiach” refers to Kotowice. I then see „10.12.1908 4545/89”. Can you help me understand how you determined that this information indicates her death in 10.12.1989?

Sorry for the slow reply. I have not visited this site in a while. The email address for USC Siemianowice Slaskie is usc@um.siemianowice.pl. Ms. Gansiniec replied to my request. After making my specific request she replied with information about what was in their records. I was also asked to establish, with documentation, who I was and how I was related to the people whose records I was requesting. After transferring some money to their account they send me the requested documents be traditional mail. I will say that I have made a follow up request for additional information and I have had no reply whatsoever.

UżytkownikI had some very good news from AP Katowice. They do have the records for my great aunt Gertrude Kolassa. Thanks to all that helped me get this far. The letter from AP Katowice tells me that there is a charge of 2 PLN for the cost to email to me the scans of the document. The same letter provides a lot of bank routing information about where to send the money. Does any of you know what is the best way to transfer those funds to AP Katowice? Normal bank wire transfer or even PayPal cost a lot more than the 2 PLN fee.

UżytkownikAlthough most of the ancestors of my branch of the Kolassa family seem to originate from the vicinity of Zülz (Biala), my grandmother’s siblings were all born in Laurahütte (Siemianowice Sl.) and her parents were married there. The brief span of records that have been scanned for Laurahütte (Siemianowice Sl.) include the period of time in which my great grandmother, Pauline Piontek, would likely have been born. I looked through those records and found the birth of Johanna Pauline Piątek (see attached image). The name „Piątek” is not clearly visible so I have included a second image of the record of a sibling that clearly shows the „Piątek” spelling. Most of the entries do not seem to use Polish alphabet characters in the spelling so the inclusion of the „ą” character seems a bit unusual. In all the other documents that I have seen that relate to Johanna Pauline her last name is spelled „Piontek”. So, my question is regarding the likelihood that the name „Piontek” is just a German phonetic spelling of „Piątek” or are these truly just different last name.
